Exam Proctoring Request Form 

Orillia Public Library supports lifelong learning, and offers an exam proctoring service for the following fees: $30 for up to three hours for Library members in good standing and $50 for non-library members. The costs to return an exam is $10 for return via courier/express post. 

An assigned staff member will check on students periodically, but currently does not invigilate exams.  

It is the student’s responsibility to ensure that the service provided by the Library meets the requirements of the institution or company and to ensure that exams are received in time for the scheduled appointment.

The Library will not be liable for any missing items, papers, samples or other documents related to the exam.

Please note, proctoring services are offered from Monday - Thursday between the hours of 10:00 am - 4:00 pm only. 

By submitting this application, I confirm that I have reviewed and acceppted the conditions of the Orillia Public Library's Proctoring Servicers Policy. I assume all responsibility for ensuring that the policies and guidelines required by my school are met when taking an exam at the Orillia Public Library.
